The City of Hendersonville's Request for Information Form can be downloaded below. Please be as detailed/specific as possible when filling out the application. Be sure to include both your phone and e-mail address and to sign line# 9. If you are requesting a personnel file, you will also need to complete the Request To View Personnel File Form in addition to the general form. Once completed, forms and proof of your TN residency (i.e., State ID) can be brought to the Hendersonville City Hall Finance Dept. or e-mailed to recordsrequest@hvilletn.org.
