Verifying Dependents for Insurance

City of Hendersonville requires employees to provide documentation to verify coverage eligibility for dependents added to health benefits during the Open Enrollment process or for a Qualifying Life Event. When you enroll online, you must also submit dependent eligibility verification. See table below or CLICK HERE for acceptable documents for verifying dependents.


 If you want to add a dependent for coverage, you will need to provide spouse/dependent’s:

  • Social Security #
  • Date of birth 
  • Gender
  • Address of eligible dependents if they do not reside with you
  • Relationship of eligible dependents to you

If you are electing Supplemental Life Insurance or contributing to 457B plan(s), you need to enter the beneficiary:

  • Name
  • Relationship to the employee
  • If your beneficiary is a non-person entity, then you will need the entity’s tax id or a Trust number
