City of Hendersonville requires employees to provide documentation to verify coverage eligibility for dependents added to health benefits during the Open Enrollment process or for a Qualifying Life Event. When you enroll online, you must also submit dependent eligibility verification. See table below or CLICK HERE for acceptable documents for verifying dependents.
OPEN ENROLLMENT: VERIFYING DEPENDENTS & BENEFICIARIES
If you want to add a dependent for coverage, you will need to provide spouse/dependent’s:
- Social Security #
- Date of birth
- Address of eligible dependents if they do not reside with you
- Relationship of eligible dependents to you
If you are electing Supplemental Life Insurance or contributing to 457B plan(s), you need to enter the beneficiary:
- Relationship to the employee
- If your beneficiary is a non-person entity, then you will need the entity’s tax id or a Trust number
